Thank you for choosing FADE!

Please ensure that you have read and fully understood the policies below on which Fade Medical Aesthetics provides service to its clients to booking any treatment.

APPOINTMENT AND CANCELLATION POLICY

We ask that our clients respect our 24 hour cancellation policy.  We understand that things come up and unexpected events happen in our lives.   If you are unable to attend your appointment, please contact us immediately at 306-527-6679. We will need to know that you wish to cancel 24 hours before your appointment is due.  Failure to cancel or rearrange with 24 hours notice to the scheduled time and the appointment time cannot be filled, 50% of total service value will be charged.  Failure to not arrive for an appointment without notifying Fade Medical Aesthetics will be subject to a charge of 100% of scheduled service value.  Please arrive for your appointment at least 10 minutes in advance so that the necessary paperwork can be completed.  Late arrival may result in reduced treatment time or forfeiting of the appointment.  Fade Medical Aesthetics will strive to ensure that your appointment runs to time.  However, should we need to cancel or postpone your appointment at short notice, we will make every effort to contact you in advance.  Prior to your appointment we will inform you of any preparation required in advance of your treatment.  Failure to follow the guide lines may result in cancellation of your appointment, reduced treatment time, or additional fees being charged.

CREDIT CARD DETAILS, DEPOSITS AND PAYMENTS

To secure an appointment with Fade Medical Aesthetics a deposit may be required.  For any subsequent individual treatments up to the value of $300 a deposit of $25 will be charged and for any subsequent individual treatments over the value of $300, a deposit of $50 will be charged at the time of booking.  Deposits can be used towards payment for treatment or left on your account for future treatments.  If there is a "no show" appointment for microneedling, botox, filler or blekyra, the deposit will be forfeited and another deposit will be required prior to future bookings of these servicecs.  Should deposits remain unclaimed by the client following twelve months of inactivity, Fade Medical Aesthetics reserves the right to withhold the deposit.  These charges will not be applied to clients booking an appointment from their existing package of treatment  Special offers or discounted pricing quotes are valid until stated on offer or for 3 months from quotation only.



CONSULTATIONS

Please note that a consultation is required prior to all initial treatments.  At Fade, we pride ourselves in providing the best outcomes for our clients by addressing all of your concerns, with the best customized treatment and home care regimes possible.

COURSES OF TREATMENTS

All treatments purchased at Fade Medical Aesthetics as a course must be paid for in full in advance of the first treatment.  All treatment courses must be taken within 12 months of the date of purchase.  Any treatments left untaken after 12 months will be forfeited.  Treatment plans are only refundable for medical reasons.  Any refund agreed is calculated by deducting the full list price of all treatments already taken, plus any charged for non-attendance, from the total price of the course of treatment, with the difference returned to you.

TREATMENT SUITABILITY

A Fade Medical Aesthetics, physician, nurse or medical aesthetician will always assess whether treatment is suitable for you, or likely to be successful, prior to any treatment being carried out.  If not, we will inform you as to the reasons why.  You will only be liable for the cost of the initial consultation, where applicable.

LIABILITY

It is the client's responsibility to ensure that he or she provide Fade Medical Aesthetics with all relevant medical details prior to each treatment.  Fade Medical Aesthetics will not be liable for any damage that occurs as a result of the client's failure to disclose such details.  The client agrees to comply with all instructions and/or recommendations given to them by, or on behalf of, Fade Medical Aesthetic regarding the care of a treated area.  

YOUR RIGHT TO COMPLAIN

Fade Medical Aesthetics strives to treat all its clients compassionately, appropriately and fairly.  If, however, you have an issue with any matter in relation to your treatment at Fade Medical Aesthetics, you are entitled to file a complaint, either verbally, by telephone or in writing.  If you require assistance with making your complaint, Fade Medical Aesthetics will be pleased to help.

RETURN/ EXCHANGE POLICY

At Fade Medical Aesthetics, we guarantee the products we sell.  If a product has been opened, it may be returned within 30 days of purchase date for exchange or store credit.  If a product is unopened, it may be returned for a full refund within 30 days of purchase date.
