Please ensure that you have read and fully understood the policies below on which Fade Medical Aesthetics provides service to its clients to booking any treatment.
APPOINTMENT AND CANCELLATION POLICY
Fade Medical & Aesthetics Ltd. Cancellation Policy requirements to Book an Appointment to secure an appointment with Fade Medical & Aesthetics a deposit is required. When you schedule your appointment with us, you agree to our policies. All of our services require a credit card to guarantee your appointment reservation. This includes services intended to be paid for with a gift card. Please have your credit card ready when booking online. Deposits can be used towards payment for treatment or left in your account for future treatments. Should deposits remain unclaimed by the client following twelve months of inactivity, Fade Medical & Aesthetics Ltd. reserves the right to withhold the deposit. These charges will not be applied to clients booking an appointment from their existing package of treatment. In the event of a late cancellation or failure to attend an appointment, a charge will be applied to the credit card. If you are unable to provide credit card information or if you decline to provide credit card information, you may book an appointment with an e-transfer deposit of $150.00 CDN to Fade Medical & Aesthetics Ltd. The e-transfer of funds must be received by Fade Medical & Aesthetics Ltd. a minimum of 48 hours before the appointment is booked, or the appointment will be cancelled. All e-transfers can be sent to firstname.lastname@example.org. Please arrive for your appointment at least 10 minutes in advance so that the necessary paperwork can be completed. Fade Medical & Aesthetics Ltd. will strive to ensure that your appointment runs to time. However, should we need to cancel or postpone your appointment at short notice, we will make every effort to contact you in advance. Prior to your appointment we will inform you of any preparation required in advance of your treatment.
Late Cancellation and No-Show Policy
Once you book your appointment, time with our service providers will be personally reserved for you. Please understand that when you forget or cancel your appointment without giving enough notice, we miss the opportunity to fill that appointment time and guests on our waiting list miss the opportunity to receive treatment. As a courtesy, our appointments are confirmed via text 72 hours in advance. It is ultimately your responsibility to remember and attend your appointment.
Please review the following late cancellation & no-show policy information:
A 100% treatment charge will be applied in the case of late cancellation or a no-show. * As a courtesy, consultations & certain follow-up appointments are complimentary. However, our service providers’ time is valuable, and the fee will still apply in the case of a no-show or late cancellation for these services. If you are 15 minutes or later for your appointment time without notifying us, your appointment will be considered a no show and applicable fees will apply.
Please call Fade Medical & Aesthetics at (306) 527-6679 if you are unable to attend your appointment. We request that you give us at least 48 hours’ notice. This policy is in place so that we can offer everyone the earliest and most convenient appointment possible. By booking an appointment at Fade Medical & Aesthetics Ltd. you agree to the terms of our Cancellation and No-Show Policy.
We ask that you please reschedule or cancel at least 2 days before the beginning of your appointment or you may be charged a cancellation fee.
COURSES OF TREATMENTS
All treatments purchased at Fade Medical & Aesthetics as a course must be paid for in full in advance of the first treatment. All treatment courses must be taken within 12 months of the date of purchase. Any treatments left untaken after 12 months will be forfeited. Treatment plans are only refundable for medical reasons. Any refund agreed is calculated by deducting the full list price of all treatments already taken, plus any charged for non-attendance, from the total price of the course of treatment, with the difference returned to you.
A Fade Medical & Aesthetics, physician, nurse or medical aesthetician will always assess whether treatment is suitable for you, or likely to be successful, prior to any treatment being carried out. If not, we will inform you as to the reasons why. You will only be liable for the cost of the initial consultation, where applicable.
YOUR RIGHT TO COMPLAIN
Fade Medical & Aesthetics Ltd.. strives to treat all its clients compassionately, appropriately and fairly. If, however, you have an issue with any matter in relation to your treatment at Fade Medical & Aesthetics Ltd, you are entitled to file a complaint, either verbally, by telephone or in writing. If you require assistance with making your complaint, Fade Medical & Aesthetics Ltd. will be pleased to help.
RETURN/ EXCHANGE POLICY
At Fade Medical & Aesthetics Ltd. we guarantee the products we sell. All sales are final at Fade Medical & Aesthetics Ltd.
It is the client’s responsibility to ensure that he or she provide Fade Medical & Aesthetics Ltd. with all relevant medical details prior to each treatment. Fade Medical & Aesthetics Ltd. will not be liable for any damage that occurs as a result of the client’s failure to disclose such details. The client agrees to comply with all instructions and/or recommendations given to them by, or on behalf of, Fade Medical & Aesthetics Ltd. regarding the care of a treated area.
